Accountable care organizations were designed by CMS to provide incentives to increase the quality of care while reducing its cost. They also allow participants to share in some of the savings generated by this model. A key element is to manage patients from a preventive perspective by getting more insights into their care, anticipating their needs, and slowing their disease progression. This adds value to both the facility and its patients. By understanding and anticipating their needs, caregivers can optimize their patients’ health and improve their quality of life.

This presentation identifies the key elements in leading a successful ACO to improve quality, reduce healthcare costs and enable you to share in the savings that you generate.

By attending this webinar, participants will be able to:

  • Describe how ACOs contribute to Value-Based Care
  • Name the key elements in ACO success
  • Explain the value in documenting risk
  • Identify the benefits of transitional care management and ED follow-up

ACO Implementation – A Team Sport

In order to succeed in an Accountable Care Organization (ACO), all participants in the facility need to be aware of the goals and objectives of joining an ACO, and each individual needs to understand what role they play in the success of the ACO. Members of the team should be engaged both in the design of the implementation and the potential reward for participation. Changing one’s practice to meet the requirements of an ACO is not without challenges. Consequently, staff and providers need to believe it will add value both to their patients’ lives and to theirs.

Traditionally, one of the more challenging groups to engage is the providers. They already have many demands on their time and adopting new ways of doing things requires a vision of why it is important and why it will benefit them and their patients.

This presentation will describe some of the elements that help build a team to successfully manage this transition so that everyone succeeds.

By attending this webinar, participants will be able to:

  • Describe the steps to engage your team
  • Name the qualities necessary for a successful Physician Champion
  • Explain the process of involving the team in ACO participation planning
  • Identify the role each team member plays in the success
  • Describe ways to celebrate and reward changes needed for success
