Rural Healthcare Provider Transition Project (RHPTP) is designed to help strengthen value-based care (VBC) for health care organizations by building the recipient’s foundation for VBC in efficiency, quality, patient experience, and safety of care.
As RHPTP wraps up technical assistance (TA) for the 2020-2021 project year, the National Rural Health Resource Center (The Center) is excited to announce that the 2021-2022 RHPTP Application is now open! To be considered, applications must be completed in their entirety by 11:59 PM CST on September 10, 2021.
