Join the Health Quality Innovation Network’s (HQIN) new MedsMatter! series featuring learning and action events to help you implement key medication strategies in your setting and strengthen medication safety in your community. Sessions will include one or more of the following:
- Education and practical examples of medication safety programs
- Tips from organizations implementing these programs
- Ready-to-use resources for quick implementation, reimbursement and sustainability
- Follow-up content and community connections
During the first session, you will learn about Medication Therapy Management (MTM) – what it is, how it works, turnkey programs to use and how MTM is integrated into Chronic Care Management and Transitional Care Management programs. You also will learn how providers are conducting MTM successfully, especially during COVID-19, and get tips and resources to maximize your processes and “connect the dots” for patient safety during care transitions.
