May 18, 2020

The Centers for Medicare & Medicaid Services (CMS) is excited to release a brief on “Partnering for Impact: Early Insights from the Accountable Health Communities Model.” The Accountable Health Communities (AHC) Model assesses whether bridging the gap between clinical care and social services can reduce health care utilization and costs for high risk Medicare and Medicaid beneficiaries. Learn more about their insights in the brief “Partnering for Impact: Early Insights from the Accountable Health Communities Model.”
