Many health care providers are leveraging the flexibility of value-based payment models to address social determinants of health (SDOH) while improving the value and quality of care. The movement toward addressing SDOH, such as transportation, nutrition, and housing, recognizes the important role these nonmedical needs play in a patient’s health and well-being.
This session highlights how some rural health providers are collaborating with community-based organizations to address SDOH needs that result in improved health outcomes, reduced costs, and advancing health equity.
