Missouri policy allows pharmacists to provide easier access to free or low-cost COVID-19 testing

A standing order was issued Tuesday, March 15, 2022, by Missouri’s State Epidemiologist, Dr. George Turabelidze, which entitles individuals to receive coverage for purchased tests for SARS-CoV-2, the virus that causes COVID-19. This policy authorizes Missouri pharmacies to charge an individual’s insurance carrier, whether it is private insurance, Medicaid or Medicare coverage, for COVID-19 testing.

“Those without a healthcare provider or who lack immediate access to healthcare will have the opportunity to seek testing close to home, for free or at a reduced cost,” said Paula Nickelson, Acting Director of the Missouri Department of Health and Senior Services. “Dr. Turabelidze’s action is providing one more way we can ease the burden on consumers and the healthcare industry while also limiting spread of the virus in our communities.”

Testing options for COVID-19 have evolved considerably since the start of the pandemic and it remains a very important tool for limiting spread, and therefore decreasing instances of hospitalization and death.

From testing to vaccinations, pharmacists have played a vital role in Missouri during the COVID-19 pandemic. With Missouri’s broad network of pharmacists, these providers have localized COVID-19-related services, increasing accessibility to individuals in all areas of the state.

“The Missouri Pharmacy Association is excited for this standing order, as this will help pharmacists aid their communities in ensuring tests can be utilized to limit the spread of this virus,” said Ron Fitzwater, Chief Executive Officer of the Missouri Pharmacy Association.

RHC Survey Regarding Interest in COVID-19 Therapeutics

The Health Resources and Services Administration (HRSA) is considering adding a limited supply of antiviral drugs to the suite of HRSA direct supply programs for Rural Health Clinics (RHCs) to treat clinically eligible patients who test positive for COVID-19!

In order to gauge RHC interest and need for therapeutics, HRSA is requesting that RHCs complete the brief, 5-question survey found here by 11:59 p.m. your local time on Tuesday, March 2.

As a reminder, RHCs can enroll in the ongoing RHC Test Supply Program to obtain direct access to free at-home test kits, N95 masks, and point of care testing supplies via the DPOP system. You can find directions for enrolling here.

Completing this survey does not enroll your RHC in this program should supplies become available and is only intended to gauge interest and need.

NARHC Webinar – Ordering Over-The-Counter Tests from HRSA

In December, the Health Resources and Services Administration (HRSA) announced the launch of the HRSA COVID-19 Testing Supply Program. Rural Health Clinics RHCs) are able to enroll and request over-the-counter COVID tests be shipped directly to RHCs or designated sites across the country. Hundreds of RHCs have successfully enrolled and received over-the-counter COVID testing supply.

The National Association of Rural Health Clinic (NARHC) will be hosting a webinar on Monday Feb. 14th at 2pm Eastern to provide a status update on the HRSA COVID-19 Testing Supply Program and discuss how to properly enroll in the Health Provider Ordering Portal (HPOP) and the Diagnostic Provider Ordering Portal (DPOP).

Some RHCs have expressed issues with enrollment and registration due to extremely high demand for over-the-counter COVID tests and quick launch of the program for RHCs. HRSA has been extremely dedicated to resolving issues as fast as possible and has funded NARHC through a cooperative agreement to help RHCs through the enrollment and ordering process.

Biden-Harris Administration Will Cover Free Over-the-Counter COVID-19 Tests Through Medicare

CMS Developing Initiative to Enable Access to Eight Free Over-the-Counter COVID-19 Tests for Medicare Beneficiaries in Early Spring

As part of the Biden-Harris Administration’s ongoing efforts to expand Americans’ access to free testing, people in either Original Medicare or Medicare Advantage will be able to get over-the-counter COVID-19 tests at no cost starting in early spring. Under the new initiative, Medicare beneficiaries will be able to access up to eight over-the-counter COVID-19 tests per month for free. Tests will be available through eligible pharmacies and other participating entities. This policy will apply to COVID-19 over-the-counter tests approved or authorized by the U.S. Food and Drug Administration (FDA).

This is the first time that Medicare has covered an over-the-counter test at no cost to beneficiaries. There are a number of issues that have made it difficult to cover and pay for over-the-counter COVID-19 tests. However, given the importance of expanding access to testing, CMS has identified a pathway that will expand access to free over-the-counter testing for Medicare beneficiaries. This new initiative will enable payment from Medicare directly to participating pharmacies and other participating entities to allow Medicare beneficiaries to pick up tests at no cost. CMS anticipates that this option will be available to people with Medicare in the early spring.

Until then, people with Medicare can access free tests through a number of channels established by the Biden-Harris Administration. Medicare beneficiaries can:

  • Request four free over-the-counter tests for home delivery at covidtests.gov.
  • Access COVID-19 tests through healthcare providers at over 20,000 free testing sites nationwide. A list of community-based testing sites can be found here.
  • Access lab-based PCR tests and antigen tests performed by a laboratory when the test is ordered by a physician, non-physician practitioner, pharmacist, or other authorized health care professional at no cost. In addition to accessing a COVID-19 lab test ordered by a health care professional, people with Medicare can also already access one lab-performed test without an order, also without cost sharing, during the public health emergency.

In addition:

  • Medicare Advantage plans may offer coverage and payment for over-the-counter COVID-19 tests as a supplemental benefit in addition to covering Medicare Part A and Part B benefits, so Medicare beneficiaries covered by Medicare Advantage should check with their plan to see if it includes such a benefit. 
  • All Medicare beneficiaries with Part B are eligible for the new benefit, whether enrolled in a Medicare Advantage plan or not.

HRSA Takes Action to Fight the Pandemic, Improve Health Outcomes and Promote Health Equity

In January, the Health Resources and Services Administration (HRSA) announced new leadership, $103 million in new resources to reduce health care provider burnout, $2 billion in pandemic relief payment to health providers, new no-cost preventive benefits for women and children, and new funding to combat the opioid epidemic.

In January, HRSA, the division of the U.S. Department of Health and Human Services devoted to improving health outcomes in underserved communities, promoting health equity, and supporting the health workforce, took the following actions:

HHS Secretary Xavier Becerra Welcomes Carole Johnson Back to HHS as New HRSA Administrator

  • HRSA’s new Administrator, Carole Johnson, took office with a commitment to advancing health equity, supporting the health workforce, and improving health outcomes, particularly in underserved and rural communities.

Biden-Harris Administration Awards $103 Million in American Rescue Plan Funds to Reduce Burnout and Promote Mental Health and Wellness Among Health Care Workforce

  • HRSA awarded $103 million in American Rescue Plan funding to 45 grantees to improve the retention of health care workers and help respond to the nation’s critical staffing needs by reducing burnout and promoting mental health and wellness among the health care workforce. Awards will fund evidence-informed programs, practices, and training, with a specific focus on providers in underserved and rural communities.

HRSA Updates the Affordable Care Act Preventive Health Care Guidelines to Improve Care for Women and Children 

  • HRSA updated the comprehensive preventive care and screening guidelines for women and for infants, children, and adolescents that, under the Affordable Care Act (ACA), certain group health plans and insurance issuers must cover with no out-of-pocket cost. Updates include, for the first time, covered group health plans and insurance plans must provide coverage without a co-pay or deductible for double electric breast pumps and adding screening for suicide risk for adolescents.

HHS Distributing $2 Billion More in Provider Relief Fund Payments to Health Care Providers Impacted by the COVID-19 Pandemic

  • HRSA made more than $2 billion in Provider Relief Fund (PRF) Phase 4 General Distribution payments to more than 7,600 providers across the country. These payments come on the heels of the nearly $9 billion in funding released in December 2021. With the January announcement, a total of nearly $11 billion in PRF Phase 4 payments has now been distributed to more than 74,000 providers. This is in addition to HRSA’s distribution of American Rescue Plan (ARP) Rural payments totaling nearly $7.5 billion in funding to more than 43,000 providers in December 2021.

HHS Announces Availability of $13 Million to Increase Behavioral Health Care Access in Rural Communities

  • HRSA announced the availability of $13 million in funding to increase access to behavioral health care services and address health inequities in rural America, including through evidence-based, trauma-informed treatment for substance use disorder, including opioid use disorder.

  • HRSA released a fact sheet highlighting the agency’s 2021 investments in supporting and growing the health workforce, including supporting the largest cohort of health care providers receiving scholarship and loan repayment support in return for a commitment to practice in underserved communities.

Spread the Word About Vaccine Boosters

The U.S. Department of Health & Human Services released new resources – posters, flyers, videos, and talking points – to help promote the extra protection from COVID-19 boosters. All vaccinated adults aged 18+ are eligible for a booster. A few weeks ago, the Centers for Disease Control and Prevention (CDC) expanded booster eligibility to include adolescents ages 12 to 17, recommending that they receive a booster shot five months after their initial vaccination. The CDC also released a new resource, based on input from rural health departments and organizations, with 12 strategies to increase vaccine uptake in rural communities (pdf). Search by zip code to find nearby locations providing adult and pediatric vaccines and boosters for COVID-19 and the flu at vaccines.gov.

The LC&FIRP ECHO is intended for primary care providers but patient lived experience experts and advocates are welcomed.

Family Health Centers of San Diego, the ECHO Institute, University of Washington, and University of Colorado have collaborated to provide a Centers for Disease Control and Prevention (CDC)-funded monthly webinar-style ECHO learning session to rapidly disseminate Post-acute Sequelae of SARS-COV-2 infection (PASC) and Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) findings and emerging best practices. This large-scale national monthly ECHO webinar series will offer didactic presentations by subject matter experts, examples of emerging best practices and models of care, and a facilitated Questions & Answers. This program is open to all healthcare professionals and all Long COVID and ME/CFS patient-lived experience experts interested in learning more about the treatment of Long COVID and ME/CFS.

RHC Vaccine Confidence Grantees Must Attend Upcoming Quarterly Review Call

The National Association of Rural Health Clinics (NARHC) looks forward to seeing all Health Resources and Services Administration (HRSA) Vaccine Confidence Grantees next week for the second mandatory Quarterly Review call.

This quarter, there will be three separate calls with different areas of focus. Each Zoom meeting will feature a different Rural Health Clinic Vaccine Confidence (RHCVC) grantee, who will present on how they have used the RHCVC grant within their communities!

Additionally, HRSA staff will host a Q&A session. Grantees are required to attend at least ONE session; however, you are welcome to attend more than one session if interested in additional topics.

As a reminder, RHCVC grantees were awarded $49,500 in July 2021. Unlike other RHC funding distributions, this was not deposited into RHC bank accounts. Instead RHCs can draw down on funds using the HRSA Payment Management System (PMS). HRSA knows that many grantees have not yet drawn down on their funds and wants to ensure that each grantee feels adequately prepared to utilize this grant as funds must be spent by June 30, 2022.

NARHC recognizes the challenges associated with managing a federal grant, especially for many first time grantees, but are here to support you as effective and responsible stewards of the grant dollars to encourage future grant opportunities for all RHCs.
