In September, 2023, the Centers for Medicare & Medicaid Services (CMS) announced a new voluntary state Total Cost of Care (TCOC) model, the stated Advancing All-Payer Health Equity Approaches and Development (AHEAD) model.

The goals of AHEAD are to:

  • Bend the healthcare cost curve,
  • improve population health, and
  • advance health equity by reducing disparities in health outcomes.

AHEAD builds on other state models including the Pennsylvania Rural Health Model (PARHM).

National Rural Health Resource Center Now Accepting Applications – Rural Healthcare Provider Transition Project

The National Rural Health Resource Center is now accepting applications for the Rural Healthcare Provider Transition Project which provides selected small rural hospitals and certified RHCs with technical assistance to strengthen key elements of value-based care.

Ideal Candidates:

All eligible Small Rural Hospitals (SRHs) and Rural Health Clinics (RHCs) are encouraged to apply. Ideal applicants are those that demonstrate:

  • Need and readiness for comprehensive TA
  • A level of financial stability
  • A strong, stable leadership committed to active engagement in all phases of the project

This project will best support candidates that are not currently or have not previously received similar TA and do not have readily available support and access to VBC transition tools and resources.
