Allevant, a joint venture between Mayo Clinic and Select Medical, helps Critical Access Hospitals by developing post-acute Transitional Care programs using available Swing Beds. Focused on assisting patients to transition to their highest level of independence, Transitional Care is a program that involves the patient and family, makes available the total resources of the Critical Access Hospital, and focuses on measured outcomes and evidence-based processes to ensure patients get the best care possible.

Allevant has supported approximately 100 Critical Access Hospitals across 22 states, including independent and part of health systems. Our clients have cared for over 12,000 patients, representing over 158,000 swing bed days.

With a strong focus on quality, outcomes, and growth, our clients saw an average increase in swing bed days of 55% by the second year. 64% of patients were categorized as complex medical or complex surgical, a growing patient population. Only 7% of Transitional Care patients were discharged directly back to acute care during a program stay of fewer than 30 days. 76% of patients were discharged from Transitional Care back to independence. The average patient rating was 4.83 out of 5 when asked if patients would “recommend the program to others.”

Robert Wood Johnson Foundation Call for Applications: the Culture of Health Prize: Communities Leading the Way is open for applications through March 29, 2023. The Prize will recognize up to 10 communities with an unrestricted $250,000 award and national recognition. RWJF will award this year’s Prize to selected communities that are:

  •  Addressing structural racism and other structural injustices to create conditions that advance health equity.
  • Committing to sustainable policy, systems, environmental, and cultural changes.
  • Working alongside partners across sectors, and elevating the expertise and solutions held by people with firsthand experiences of health inequities.
  • Engaging in cultural work that celebrates community and envisions and advances a more just future.

Missouri Immunization Coalition Survey

The Missouri Immunization Coalition (MIC) is a 501(c)(3) nonprofit existing to reduce the spread of vaccine-preventable diseases through education, advocacy, promotion, and collaborative partnerships. We also coordinate with partners across the state that share our vision of better health and well-being for all, including our public health and healthcare workforce.

This is the fourth survey MIC has created to learn about the experiences and needs of our partners. Focusing on staffing challenges during the COVID-19 pandemic, the goal of this survey is learning how MIC can advocate for solutions on local, state, and federal levels with policy and funding decision-makers.

The purpose of this 5–7-minute survey is to understand how providing care for COVID-19 patients, coordinating access to COVID-19 vaccines, and promoting education about COVID-19 disease transmission and prevention has exacerbated pre-pandemic staffing shortages and how agencies are addressing the shortages.

USDA Launches Rural Data Gateway to Strengthen Partnerships and Expand Access to Resources in Rural

The U.S. Department of Agriculture (USDA) Rural Development unveiled the Rural Data Gateway, a new resource to make the Department’s data for rural projects easier to access for its partners and people living in rural communities. The Rural Data Gateway will help USDA better target resources to those who most need them in rural places. It also will help the Department be a stronger partner to rural people, entrepreneurs, government officials and other stakeholders.

The Biden Administration announced the COVID-19 Public Health Emergency (PHE) Declaration is anticipated to end on May 11, 2023. This will significantly impact CMS Certified Rural Health Clinics (RHCs) and Federally Qualified Health Centers (FQHCs).
