Missouri Primary Care Association (MPCA) along with Missouri Hospital Association, Missouri Department of Health and Senior Services, National Kidney Foundation, and Harvesters are pleased to announce registration for the Five Part Health Equity and Social Determinants/Drivers of Health Learning Collaborative webinar series is now open.

This series will cover information on innovative data platforms currently being used in community health needs assessments and implementation strategies to identify where disparities exist, thus helping communities target interventions with the needed specificity. Attendees also will learn about the importance of leveraging technology to connect a multidisciplinary team to create an environment of networking and shared learning with the sole purpose of promoting the health and well-being of everyone.

The Learning Collaborative series will run May-September 2023, with sessions held monthly from May 3 through September 6 and is free to participate but registration is required.

The Missouri Health Association (MHA) is hosting a five part webinar series – Health Equity and Social Determinants of Health Learning Collaborative: A Population Health, Team-Based Care and Community-Based Approach. This webinar series meets people where they are in their health equity and social determinants of health journey.

Topics include the impact of justice, equity, diversity and inclusion on health equity and SDOH. Attendees will hear statewide, regional and local standpoints related to how health equity and SDOH currently are addressed.

Additional highlighted topics will include standardized SDOH screening tools, innovative data platforms used in community health needs assessments and implementation strategies, and social referral platforms being used in Missouri.

Allevant, a joint venture between Mayo Clinic and Select Medical, helps Critical Access Hospitals by developing post-acute Transitional Care programs using available Swing Beds. Focused on assisting patients to transition to their highest level of independence, Transitional Care is a program that involves the patient and family, makes available the total resources of the Critical Access Hospital, and focuses on measured outcomes and evidence-based processes to ensure patients get the best care possible.

Allevant has supported approximately 100 Critical Access Hospitals across 22 states, including independent and part of health systems. Our clients have cared for over 12,000 patients, representing over 158,000 swing bed days.

With a strong focus on quality, outcomes, and growth, our clients saw an average increase in swing bed days of 55% by the second year. 64% of patients were categorized as complex medical or complex surgical, a growing patient population. Only 7% of Transitional Care patients were discharged directly back to acute care during a program stay of fewer than 30 days. 76% of patients were discharged from Transitional Care back to independence. The average patient rating was 4.83 out of 5 when asked if patients would “recommend the program to others.”
